Use this form to request changes to club contact information in the SDLTA directory.
Contacts to remove
List the names and/or roles of contacts that should be removed.
Contacts to add
Provide the full details for each new contact to be listed.
Note: To enter results, captains first need to register as a Club Contact with the LTA — follow this guide to get set up. If you change your club captain here, remember to update the LTA too, or the new captain won't be able to enter results.
